The Importance of Patient Engagement: Insights from Greenbrook Medicalʼs Neil Machhar

Patient engagement isn’t just a buzzword in healthcare—it’s a critical indicator of health outcomes. On the latest episode of Healthcare Ops Wave, Third Way Health CEO Frederik Mueller sat down with Neil Machhar, Co-founder and CEO of Greenbrook Medical, to discuss the evolving landscape of patient engagement, value-based care, and sustainable healthcare operations.
A Patient-Centered Approach to Primary Care
Greenbrook Medical was founded in 2021 with a clear mission: to provide high-touch, personalized primary care for seniors. While many healthcare startups chase rapid growth and aggressive expansion, Greenbrook Medical has taken a different approach—focusing on patient engagement and operational excellence before scaling.
“
For us, engagement with our patients is one of the most important indicators of their health outcomes,”
Machhar explained. “It shows they trust us, and more importantly, that they are actively involved in their
own
care.”
Measuring engagement, however, is no simple task. Greenbrook Medical prioritizes:
- Tracking patient visits and touchpoints
- Aligning team incentives around engagement
- Ensuring rapid access to care for new patients
- Monitoring completion rates for annual wellness visits
This data-driven approach ensures accountability and helps optimize the patient experience.
Building a Sustainable Growth Model
Unlike many venture-backed healthcare startups, Greenbrook Medical has bootstrapped its growth. “We
deliberately chose not to raise money early on,” Machhar said. “We wanted to prove the model before setting
expectations with investors.”
This careful, measured approach has led to:
- Three clinic locations with 25 employees
- A Net Promoter Score (NPS) of 96
- Profitable operations with strong clinical outcomes
Greenbrook Medical’s decision to stay lean and prioritize quality over speed has helped them navigate the complexities of Medicare Advantage and risk- adjusted care models.

In-House vs. Outsourced: A Strategic Approach
A critical component of scaling effectively is deciding what to keep in-house versus what to outsource. Machhar outlined Greenbrook Medical’s guiding principles:
- Keep anything that directly impacts patient experience in-house.
- Outsource administrative and non-patient-facing functions.
- Choose vendors based on speed, execution quality, and minimal
- oversight requirements.
